3.Please complete the Chinese and English red fonts in the template according to personal identity and departure requirements, and change all fonts to black, print and ask the class teacher or grade leader to sign。

4.Students must download and fill in the "Reading Certificate" at the same timeTravel safety book(Attachment), printed byParent signature。At the same time, bring the signed "Reading Certificate" and the exit safety guarantee letter (retained by the school) to Chengzhi Building 212 for sealing procedures。
